Foundation Pharmacist Training 25/26 - Designated Supervisors and Designated Prescribing Practitioners Registration

All foundation trainee pharmacists are required to have a named Designated Supervisor (DS) prior to starting their programme as described in the training site requirements. Those on the full learning outcomes are also required to have a named Designated Prescribing Practitioner (DPP).

NHS England needs to promptly collect information on each trainee, their programme and their DS as this must be shared with the GPhC in advance of the trainee’s start date. This information is also used to grant trainees and supervisors timely access to the e-portfolio.

The registration form is below:

  • Designated Supervisors need to complete this by 7th July 2025
  • Designated Supervisors for rotations of 13 weeks or more need to complete this by 7th July 2025.
  • Designated Prescribing Practitioners need to complete this by week 13 of the programme, or at least 21 days before the trainee starts to work with them if it is before week 13

Please note: the form can only be completed by the DS/DPP themselves. The supervisors will need trainee’s name, oriel pin and the confirmed start date.

If, after submitting this registration form, there are changes to the supervisor(s) a change form will be available and new supervisors starting within the FTPP will also be required to complete the registration form.
