Care Navigation and Triage - New assessment tool for modern general practice

Shared on behalf of NHSE

New assessment tool for modern general practice and updated care navigation and triage guidance

NHS England has published a new assessment tool for modern general practice – a diagnostic tool developed and tested with 540 practices across the country. It supports practices to review their care navigation and clinical triage process, and digital workflow, to identify what is working well and to identify and prioritise areas for improvement.

The tool links to an updated practical ‘how to’ guide covering care navigation and triage and digital workflow. Based on two years of research with 230 GPs and general practice staff, the guide is designed to support practice improvement teams who want to focus on a specific area or review their process from end-to-end.

System Collaborative Programme - Open for Expressions of Interest

System Collaborative Programme

Delivering the transformation of services described in Fit for the Future: 10 Year Health Plan requires collaborative leadership across organisational boundaries.

The System Collaborative Programme is designed for teams striving to improve population health and inequalities by working across organisational boundaries and helps individuals to develop the collaborative leadership skills needed to tackle challenges, build trust, and deliver better health outcomes for their patients and communities.

In terms of impact, completion of the programme has enabled past participants to successfully progress system-wide projects while also increasing their personal leadership effectiveness:

‘I would recommend that anybody who has a project or an idea that they want to get off the ground to sign up for the programme because it’s so helpful in helping to solidify what your idea is, and move it successfully from an idea stage to an implementation stage with some really significant personal and group learning in the process.’

Who should apply?

Your collaborative can be multidisciplinary, cross-organisational, or cross-agency, and can be comprised of a number of people, or may just start with you as an individual, with a view to bringing others in as you progress. The key requirement is that you are working to lead initiatives that improve health outcomes for your patients, population and community. Past collaboratives include:

  • Improving ambulance handover
  • Improving access to paediatric palliative care
  • Empowering deprived communities
  • Improving stroke rehabilitation
  • Improving the system response to children with complex needs

The deadline for applications is Friday 24 October. Register for the programme briefing session to find out more, or download our programme pack here: System Collaborative Programme – North East and Yorkshire Leadership Academy.
