Nominations
We asked for people to highlight outstanding pieces of work happening in their Practice or PCN.
Three were chosen and given a special mention the Conference.
Brief details are below – contact us for more information.
Victoria Medical Centre
The launch of a walk in clinic now means that all patients with urgent same day needs are accommodated - no more having to ask patients to call back the next day or contact 111
Maltby and Wickersley PCN
In Nov 2023, the PCN introduced a new total triage system called Anima. This has changed the way the practices work and opened up access to patients who can use digital technology whilst also freeing up the longer queues both in practice and by telephone in the mornings.
It has been a huge undertaking as both clinicians and non clinicians have had to change the way that they work and our patients have needed to adapt.
Network North PCN
A forward thinking PCN with a strong MSK FCP service now embracing pain management services which is having fantastic outcomes for their patients.
Offloading repeated GP appointments, redirecting the management of chronic pain and enhancing patient experiences.
How to Guide: Maximising the Potential of People in Primary Care
This How To Guide was introduced at the conference by Sarah Withers, Strategic Lead: Primary Care Additional Roles Programme and AHP Advanced Practice Advisor for the Workforce & Training Hub.
The guide provides straight forward guidance to Primary Care employers in how to approach planned expansion of PCN teams. From considering the purpose and objectives of recruitment, to how to find the right person and support new colleagues in starting well.
The guide goes on to suggesting approaches to establish effective ways of working which support team work. It also includes a trouble shooting section and links to resources.
